Last week (Aug. 28 - Sept. 1) was National Community Health Worker (CHW) Awareness Week! We celebrated BIG by meeting with local mayors of areas that we serve to do a proclamation honoring the week, to spotlight the Community Health Worker Program.
The CHW program is an essential part of our Community Health Center system. The program allows our CHW staff to assist patients in different areas such as food, housing, employment, and so much MORE. Our CHWs are trained to connect patients to needed resources to bridge the social determinant gaps that can impact patient health. The vital role that CHWs play is crucial to population health and improving quality of life for patients, as these social determinants of health can have a strong impact on overall wellness.
