We take our commitment to serve Southeast Arkansas communities seriously, with 16 in-school clinics and 13 health clinics throughout the region. Our goal is to provide your family with access to convenient and high-quality healthcare. We partners with schools and community leaders to connect with families, provide free resources and giveaways, and ensure your healthcare needs are met.

Our primary goal is to provide the best comprehensive and coordinated healthcare for patients at all stages of life. That means we put you, the patient, first. From your medical team to the staff at your local clinic, our first priority is your healthcare. Together, we’ll work out a plan just for you.

Patient-Centered Medical Home

A patient-centered medical home (PCMH) is a healthcare model aimed at improving care quality, patient experience, and health outcomes by centering care around the patient. Key features include:

Comprehensive Care: Our team of healthcare providers work collaboratively to meet your physical, emotional, and social health needs, including prevention, wellness, acute, and chronic care. Mainline Health utilizes evidence-based care practices to ensure the highest level of care. 

Patient-Centered: Our care approach respects and responds to individual patient preferences, needs, and values, ensuring that patient values guide all clinical decisions.

Coordinated Care: Care is coordinated across the broader healthcare system and referrals can be made to better meet your needs. This includes specialty care, hospitals, home health care, and community services, to ensure smooth transitions and comprehensive management. Free resources are available to all of our patients for assistance with housing, transportation, workforce, and more.

Accessible Services: The PCMH model emphasizes enhanced access to care through shorter wait times for urgent needs, expanded hours, and innovative options for communication between patients and their care teams, such as telehealth and our Patient Portal. You can send secure messages to your care team, view your health records, request appointments, and more. Mainline Health also offers after-hours services at our Mainline Health Monticello Clinic every Friday from 12 PM - 5 PM and Saturday from 8 AM - 5 PM for a more convenient care experience.

Overall, by being a participating PCMH, we can provide patients with a centralized setting for all healthcare needs, while improving coordination, reducing fragmentation, and enhancing overall healthcare efficiency and effectiveness.

To further support patient-centered care, we encourage the use of our Patient Portal, which includes patient education materials and self-management support. You can also view your health records, schedule appointments, communicate with providers, request prescription refills, and fill out our Satisfaction Survey.
