School-Based Health Center Consent Form

There must be a consent form on file to see the provider at your child's school. TO MAKE AN APPOINTMENT for a school-aged child, call the school’s clinic directly. You do not need to be present at the appointment. The healthcare provider will call you following the appointment to discuss any next steps or treatment.
