If you are a previous home health patient, in need of a post hospital or ER follow up visit, it is difficult for you to visit us in clinic, or you have transportation issues, then you can sign up for an In-Home Medical Visit.
For your convenience, an in-home medical visit can be scheduled on Tuesday or Wednesday. In these visits we can address an array of issues such as post-surgery follow-ups, obtain blood for lab work, disease management, preventative medicine, acute illness, and annual wellness visits.
