Things to Know

If you would like your child to be seen at the Warren SBHC, you must fill out a consent form. This allows our staff to see and treat the child. Parents/guardians do NOT have to be present at the time of the visit, but will be contacted if the child is seen. The consent form will be kept on file and must be renewed every year.

If your child is sick and does not come to school because of illness, you may still bring your child to the school-based clinic to see a provider. If the child is in elementary, they may be accompanied by a MSHI staff member to get to and from the clinic.
