Welcome to the Mainline Health Eudora Medical Clinic, located off Route 65 in downtown Eudora. Our hours of operation are 8:00 AM – 4:30 PM Monday – Thursday, and 8:00 AM – 12:00 PM on Fridays.
Eudora is a small community, and we are the ONLY healthcare facility in the area. We are here for all your healthcare needs. We offer Primary Care and Behavioral Health services. Primary care is anything from routine/sick visits, routine bloodwork, check-ups, to wellness exams, and more. For your mental and behavioral health needs, we offer outpatient individual and group therapy sessions. Our skilled behavioral health team is trusted, and they truly enjoy making our patients happy.
We understand that there may be limited resources in the area, but we are here to help. Did you know we have a Community Health Worker Program? The purpose of the CHW Program is to provide resources to our patients and communities. If you have a need for transportation, employment and workforce, or housing, this program would be a great option. Our team includes nurses, care coordinators, dentists, and other healthcare providers to assist with the services you need. Once you join the program, you will be assigned a primary Community Health Worker to facilitate your needs. This person will use the tools available to them to find the necessary community resources needed to help you.
For Medicare patients, we also offer a Chronic Care Management Program (CCM). The CCM team is available 24/7 to assist you with scheduling appointments, transportation needs, paying for your prescriptions, disease education and management, an so much MORE. The Eudora Medical Clinic currently offers OB/GYN services. Leia O'Fallon, APRN offers exceptional care to all of her patients. Leia is in Eudora every Monday from 8AM - 12PM.
